The insurance companies don’t create these reports on their own. Instead, there are a few companies that compile a health dossier on individuals who apply for insurance.
Once the dossier is ready, a “health credit report” is sold to the insurance company that requests it. The good news is, you have the right to see your report whenever you’d like.
If you’re applying for life insurance you should know what your report looks like. If you find mistakes, for example, your report claims that you’re taking a medication that you’ve never actually been prescribed, you can have it corrected. This could save you some serious money on your policy.
Here are three of the top health credit report companies and how to contact them to learn about your information.
Milliman IntelliScript
Milliman IntelliScript has prescription information about you if you’ve authorized the release of your medical records to an insurance company and that company requests a report. To request a copy of your report, call 877-211-4816.
You will need to provide the following information when requesting your report:
- Full Name
- Mailing address (and email address if you’d like to receive your report via email)
- Your phone number (in case the company needs to call you with questions)
- Date of birth
- Last four digits of your Social Security number
- Insurance company that you have applied with
ExamOne
ExamOne is a Quest Diagnostics company. It’s one of the companies that help determine how big of a health risk you are. It gathers much of the information on you from lab tests conducted over the years.

MIB Group
MIB Group was formerly known as the Medical Information Bureau. It tracks everyone who applies for individually underwritten life, health, or disability income insurance during the previous seven years.

FYI, some of these reports could take up to 10 business days to receive so plan ahead. Also, the reports are free so it won’t cost you anything to check them out. I’d recommend looking at them even if you’re not applying for life insurance just to make sure there are no mistakes.
